What is Vasovasostomy?

Vasovasostomy is a microsurgical procedure to reverse a vasectomy by re-anastomosing the two divided ends of the vas deferens under the operating microscope. Using 10-0 nylon sutures and high magnification, the vasal lumen is precisely realigned in a two-layer or single-layer closure. Success rates depend critically on the time elapsed since vasectomy — patency rates exceed 90% within 3 years and decline to 50–70% after 15+ years. Sperm quality also diminishes with time due to anti-sperm antibody formation. The procedure is performed under general or spinal anaesthesia with a hospital stay of Day Care.

Men with previous vasectomy who wish to restore natural fertility, including those who have changed partners or lost a child. Patients must be adequately fit for anaesthesia and free from active systemic infection before proceeding.

Incision & Access

Bilateral scrotal incisions; divided vas deferens ends identified and mobilised.

Fluid Sampling

Fluid from the testicular end sampled and examined for sperm under the microscope.

Decision Point

If sperm absent from testicular end, vasoepididymostomy is planned instead.

Anastomosis

Operating microscope used; vasal lumen mucosa re-anastomosed with 10-0 nylon in a two-layer closure.

Wound Closure

Muscularis and adventitia closed in a second layer; wound closed.
