What is Vasoepididymostomy?

Vasoepididymostomy is the most technically demanding male infertility microsurgical procedure, anastomosing the vas deferens directly to the epididymal tubule proximal to the site of obstruction. It is performed for epididymal blockage from prior infection, previous vasectomy reversal failure, or congenital epididymal blockage. The anastomosis is performed using 10-0 or 11-0 nylon sutures under 25–40x magnification, matching the tiny 0.1–0.3 mm epididymal tubule to the vas lumen. The procedure is performed under general or spinal anaesthesia with a hospital stay of Day Care. Success rates reach 40–70% in appropriately selected patients at experienced centres.

Men with obstructive azoospermia from epididymal blockage — post-infective, post-vasectomy reversal failure, or congenital — where sperm are present in the epididymis proximal to the obstruction. Patients must be adequately fit for anaesthesia and free from active systemic infection before proceeding.
