Preputioplasty (Dorsal Slit + Transverse Closure)
Foreskin-Preserving Surgery to Widen the Preputial Opening
What is Preputioplasty (Dorsal Slit + Transverse Closure)?
Preputioplasty is a foreskin-preserving surgical procedure that widens a narrow preputial opening (phimosis) without removing the foreskin. A dorsal longitudinal incision is made through the tight preputial ring and closed transversely (Heineke-Mikulicz principle), immediately widening the opening. It is suitable for mild to moderate phimosis in patients who wish to retain their foreskin for cultural, religious, or personal reasons. The procedure is performed under general or spinal anaesthesia with a hospital stay of Day Care. Success rates reach >85% in appropriately selected patients at experienced centres.
How the Procedure Works
Anaesthesia & Assessment
Local or general anaesthesia administered; phimotic ring identified and assessed.
Dorsal Longitudinal Incision
Dorsal longitudinal incision made through the full thickness of the preputial ring.
Opening Widened
Incision opened out widening the preputial opening significantly.
Transverse Closure
Wound closed transversely with fine absorbable sutures (Heineke-Mikulicz principle).
Post-Op Care
Dressing applied; post-operative hygiene and care instructions provided.

"Preputioplasty offers our phimosis patients a foreskin-preserving option. When lichen sclerosus is absent and the phimosis is localised, it is an elegant solution that respects the patient's wishes."
