Artificial Urinary Sphincter — AMS 800 (males)

What is Artificial Urinary Sphincter — AMS 800 (males)?

The Artificial Urinary Sphincter AMS 800 is the gold-standard surgical treatment for moderate-to-severe male stress urinary incontinence, most commonly occurring after radical prostatectomy or radiation therapy. The device consists of a fluid-filled urethral cuff, a pressure-regulating balloon, and a control pump in the scrotum. By squeezing the scrotal pump, the patient deflates the cuff allowing urination; the cuff automatically re-inflates to maintain continence. The procedure is performed under general or spinal anaesthesia with a hospital stay of 1–2 Days. Success rates reach >85% in appropriately selected patients at experienced centres.

Men with moderate-to-severe stress urinary incontinence (>3 pads/day) after radical prostatectomy, radiation, or TURP, who have failed conservative management for at least 12 months. Patients must be adequately fit for anaesthesia and free from active systemic infection before proceeding.

How the Procedure Works

1

Perineal Incision & Urethra Exposure

A perineal or scrotal midline incision is made depending on the implantation approach; bulbar urethra dissected free.

2

Cuff Sizing

A urethral cuff sizer is passed around the circumference of the urethra to determine the correct cuff size.

3

Cuff Placement

The correctly sized cuff is placed around the bulbar urethra with its tab pointing anteriorly.

4

Balloon & Pump Placement

The pressure-regulating balloon (61–70 cmH₂O) is placed in the retropubic space; scrotal pump positioned in the dependent scrotum.

5

Connection & Deactivation

All three components connected with kink-resistant tubing using snap connectors; device deactivated for 6 weeks allowing healing.
