AMS 800 Implantation — Perineal Approach
Artificial Urinary Sphincter Implantation via Perineal Approach
What is AMS 800 Implantation — Perineal Approach?
The AMS 800 Artificial Urinary Sphincter (AUS) is the gold-standard surgical treatment for moderate-to-severe stress urinary incontinence in men, most commonly following radical prostatectomy. The device consists of an inflatable cuff placed around the bulbar urethra, a pressure-regulating balloon reservoir, and a pump placed in the scrotum. When the pump is squeezed, fluid transfers from the cuff to the reservoir, allowing the urethra to open and urination to occur. The perineal approach is the standard approach for primary AUS implantation. The procedure is performed under general or spinal anaesthesia with a hospital stay of 1–2 Days.
How the Procedure Works
Perineal Incision & Urethra Dissection
Perineal incision; bulbar urethra dissected and measured for cuff sizing.
Cuff Placement
Appropriately sized cuff placed around the bulbar urethra.
Reservoir Placement
Reservoir balloon placed in the retropubic space through a separate groin incision.
Pump & Connection
Scrotal pump implanted in the dependent scrotum; all components connected and tested.
Device Deactivation & Closure
Device deactivated for 6 weeks; wound closed; catheter placed overnight.
