What is Male Sling (AdVance / Virtue)?

The Male Urethral Sling (AdVance or Virtue sling) is a minimally invasive procedure for mild-to-moderate post-prostatectomy stress urinary incontinence. A synthetic mesh tape is placed through small perineal and inner thigh incisions to reposition and support the proximal urethra and membranous urethra, restoring the posterior urethral angle and improving sphincter coaptation. Unlike the AMS 800, the sling does not require a pump mechanism — continence is passive and immediate. The procedure is performed under general or spinal anaesthesia with a hospital stay of Day Care / 1 Day.

Men with mild-to-moderate post-prostatectomy incontinence (1–3 pads per day), intact sphincter function on urodynamics, and no prior urethral radiotherapy. Patients must be adequately fit for anaesthesia and free from active systemic infection before proceeding.

How the Procedure Works

Perineal Incision

Perineal incision; bulbar urethra and perineal body dissected to identify the correct repositioning level.

Needle Passage

Helical needles passed through inner thighs to create obturator passage.

Mesh Positioning

Synthetic mesh tape threaded through and positioned beneath the bulbar urethra.

Tensioning

Tape tensioned to achieve posterior urethral repositioning confirmed by cystoscopy.

Wound Closure

Perineal wound closed; catheter placed overnight; discharged next morning.
