Epididymectomy
Surgical Removal of the Epididymis for Chronic Epididymitis or Abscess
What is Epididymectomy?
Epididymectomy is the surgical removal of the epididymis, performed for chronic epididymitis causing persistent pain unresponsive to conservative treatment, recurrent epididymal abscess, epididymal masses, or epididymitis in a vasectomised patient causing post-vasectomy congestive epididymitis. The testis is preserved in most cases; the vas deferens is divided and the epididymis excised from the posterior surface of the testis. The procedure is performed under general or spinal anaesthesia with a hospital stay of Day Care / 1 Day. Success rates reach >85% in appropriately selected patients at experienced centres.
How the Procedure Works
Incision & Access
Scrotal incision; tunica vaginalis opened; testis and epididymis delivered.
Vas Division
Vas deferens identified and divided at the level of the globus minor.
Dissection & Mobilisation
Epididymis carefully dissected from the posterior surface of the testis.
Vessel Ligation
Blood vessels between epididymis and testis ligated and divided.
Wound Closure
Epididymis removed; testis returned to scrotum; wound closed in layers.
Outcomes
Who Needs This Treatment?
- →Definitive treatment for chronic intractable epididymal pain.
- →Eliminates source of recurrent epididymal infection and abscess.
- →Preserves the testis and testosterone production in most cases.
- →Effective for post-vasectomy congestive epididymitis causing chronic pain.
- →Day-case or overnight procedure with minimal recovery time.
- →Histology of removed epididymis confirms diagnosis and excludes malignancy.
"Epididymectomy offers lasting relief for patients with chronic epididymal pain that has failed all other measures. Careful patient selection is key — in the right candidate, it can be transformative."
