Peyronie's Disease
Fibrous scar tissue inside the penis causing painful erections, penile curvature, and in severe cases, inability to have intercourse , correctable with injections or surgery.
ABOUT THIS CONDITION
What is Peyronie's Disease?
Peyronie's disease is caused by fibrous plaques within the tunica albuginea that produce painful erections, penile curvature, shortening, and deformity. It has an active phase (painful, changing curvature) and a stable phase (pain-free, fixed curve). Treatment is deferred until curvature has been stable for at least 6 months. Dr. Vipin offers collagenase clostridium histolyticum (Xiaflex) injections for the stable phase, Nesbit plication for moderate curvature, grafting for severe curvature, and penile prosthesis implantation for cases with coexisting erectile dysfunction , restoring comfortable sexual function.

Treatment Approach
Plaque Incision / Excision Grafting
The Peyronie's plaque is incised or excised and the defect covered with a pericardial or dermal graft , used for severe curvature (>60°) and complex deformities, restoring penile length and geometry.

Assessment
Penile curvature degree photographed or measured clinically. Penile duplex Doppler assesses erectile function before surgical planning.

Active Phase Management
Vitamin E, pentoxifylline, and collagenase injections (Xiaflex) offered during the active or early stable phase. Surgery deferred until curvature stable.

Surgical Planning
Plication for curvature <60° without shortening. Grafting for severe or complex curvature. Prosthesis implantation for Peyronie's with severe ED.

Recovery
Sexual abstinence for 6 weeks after surgery. Final curvature assessment at 3 months. Most patients achieve straight, functional erection.
