What is Revision Hip Replacement?
Revision hip replacement surgery involves removing a failed primary hip replacement — acetabular cup, femoral stem, or both — and replacing them with new, often more complex components designed to provide stable fixation in bone that has been compromised by prior surgery, wear, or infection. Indications include aseptic loosening (the most common cause), polyethylene or ceramic wear, periprosthetic joint infection (requiring two-stage revision), recurrent instability and dislocation, periprosthetic fracture, and adverse reactions to metal debris. Careful pre-operative evaluation to identify the exact failure mechanism is essential before planning revision surgery.
How the Procedure Works
Pre-op Investigation
X-rays, CT scan, blood markers (CRP, ESR), and joint aspiration where infection is suspected — full failure analysis before planning.
Component Removal
Failed acetabular cup and/or femoral stem are carefully extracted, preserving remaining bone stock.
Bone Defect Reconstruction
Bone defects are classified and addressed with augments, structural allograft, reinforcement rings, or metaphyseal sleeves.
Revision Implant Fixation
Larger, fully porous-coated or cemented stems and cups provide stable primary fixation in compromised bone.
Two-Stage Protocol (Infection)
Stage 1: remove infected implant, place antibiotic cement spacer. Stage 2 (6–12 weeks later): reimplant when infection is eradicated.
